Our Mobile Physician team provides primary care services and physician directed coordination of medical requirements for seniors in their homes. Our focus is serving patients who have multiple medical conditions, more serious medical problems, chronic illnesses, and problems related to mobility. By coordinating patient care in their homes, our physicians are able to promote patient independence and quality of life, while reducing redundancy of services, medication errors, unnecessary hospitalizations, and other costly obstacles to healthy living.

Our People – Our highly experienced medical team consists of physicians who are Board-Certified in Internal Medicine or Family Practice, many of whom have obtained additional fellowship training in Geriatric medicine, and a highly trained support staff of physician assistants, nurse practitioners, and other medical professionals. Our physicians are members of the American Academy of Home Care Physicians.

Our Approach – Acting as personal health advocates, our physicians coordinate each patient's medical care across the complex health care system which may include home health care, rehabilitation, physician sub-specialists, and hospitals. We work very closely with each patient and their family to develop a personalized care plan, and to promote high standards of quality and safety. Our care planning process is driven by a compassionate doctor-patient relationship, which allows for patients and families to participate in care planning decisions. We utilize information technology to support quality patient care, performance measurement, patient education, and enhanced communication.

Our Services – Our physician-led medical team provides the coordination of medical care across the spectrum of ailments that affect’s today’s seniors. We treat patients with Diabetes, COPD, Cardiovascular Disease, Hypertention, Alzheimer’s, Dementia, Depression, and a variety of other ailments. We offer our services to patients who reside in assisted living facilities, independent care communities, and private residences where people are homebound or when home visits are a medical necessity. We accept Medicare as a primary insurance and bill Medicare directly for the patient, including for home visits when they are medically necessary. We provide on call coverage 24 hours a day seven days a week.
