Total Senior Health Care was founded as an innovative solution to address the growing challenge to deliver high quality health services to the rapidly expanding senior population. Our care coordination delivery model emphasizes the primary care physician as the central coordinating figure in the delivery of medical care, and the individual’s home as the primary location for healing. We believe that primary care physicians are uniquely qualified to serve as an individual's personal health advocate, and that "home" is typically the preferred environment for patient's to heal, as well as the cost efficient environment to deliver care.
